What’s coming up for sfCare

In order to assist organizations in effectively applying the sfCare Framework as a holistic template for quality improvement and practice change, we are developing the following resources:

  • Getting Started with Senior Friendly Care implementation package – self-assessment tool, sample sfCare policy, a training module for executive leaders and one for staff (target spring/summer 2018)
  • Senior Friendly 7 toolkit – a toolkit for healthcare providers, patients and caregivers, focusing on 7 priority areas which have a significant individual and additive impact on the independence and well-being of older adults at risk. The toolkit includes resources on: cognition/delirium, mobility/falls, nutrition, polypharmacy, continence, pain, and social engagement (target summer 2018)

We are also advocating for the development of:

Standards and indicators – we have submitted a proposal to Health Quality Ontario (HQO) for consideration to develop  a suite of senior friendly quality standards and indicators.
